Preparation makes everything go smoothly. Please click here to complete your new patient forms before your visit. On the day of your appointment, we will get started with everything necessary to assess your needs. Most first appointments last at least 90 minutes, so please plan accordingly.

If you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e. antibiotics for pre-med) please alert the office.

Please assist us by gathering the following information and bringing it to your consultation:

  • Any x-rays, if necessary. If you have a previous dentist, please instruct them to send your documents to our office at info@winfamdental.com
  • A list of medications you are currently taking
  • If you have dental insurance, please bring your cards and/or ID numbers. This will save time and allow us to help you process any claims.

THIS IS NOT DENTAL INSURANCE. Cannot be combined with other insurance or discount plans. Membership plan is specific to one person and cannot be transferred. Covered services not used within the specified time period will not be carried over or transferred. Services will be provided at this location only. Membership plan will auto-renew each year on the renewal date. If a patient presents with periodontal infection, a personalized treatment plan will be presented at an additional cost. The perio plan is strictly for preventive maintenance care.
