On the day of your appointment, we will greet you promptly and get started with everything necessary to assess your needs. Most first appointments last at least 90 minutes, so please plan accordingly.

We believe everyone at any age can enjoy the benefits of good oral health. You deserve to have a healthy and beautiful smile. We look forward to sharing happy and healthy smiles with you for years to come.

Please assist us by gathering the following information and bringing it to your consultation:

  • Any x-rays if necessary. If you have a previous dentist, please instruct them to send your documents to our office at info@winfamdental.com
  • A list of medications you are currently taking
  • If you have dental benefits or medical insurance, please bring your cards and/or ID numbers. This will save time and allow us to help you process any claims.
Patient Forms

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

If you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e. antibiotics for pre-med) please alert the office.

One-time enrollment fee of $99 for existing patients, new patients $149. Enrollment fee waived under annual membership option.

THIS IS NOT DENTAL INSURANCE. Cannot be combined with other insurance or discount plans. Membership plan is specific to one person and cannot be transferred. Covered services not used within the specified time period will not be carried over or transferred. Services will be provided at this location only. Membership plan will auto-renew each year on the renewal date. If a patient presents with periodontal infection, a personalized treatment plan will be presented at an additional cost. The perio plan is strictly for preventive maintenance care.
